Create a feedback form in just a few moments

There are options to create feedback forms from multiple locations in the platform. This is one of the most frequently completed tasks, so we have made it as straightforward as possible to create a feedback form.

Creating a feedback form - option 1

On your home page, click on the 'Start Feedback' button if you have this permission.

Providing feedback on a timetabled lesson for an individual teacher

Below is a key with descriptions of all the features available on your 'Create Feedback' form when opting to provide feedback for a teacher with a set timetable in your MIS.

1Choose to provide feedback for an individual teacher or group of teachers
2Select the teacher for whom you wish to provide feedback
3Select 'Yes' to provide feedback for a teacher with a set timetable in your MIS
4Choose the date of the lesson for which you wish to provide feedback
5Choose the relevant class from the selected teacher's list
6Select your preferred 'Feedback Category' e.g. Lesson Drop-in, Book Review, etc.

Once you have completed the 'Create Feedback' form, click on 'Submit'.

Providing feedback on a non-timetabled lesson for an individual teacher

Please note that the vast majority of primary schools will not have teachers linked to timetables in their MIS, therefore this option will need to be selected.

Below is a key with descriptions of all the features available on your 'Create Feedback' form when opting to provide feedback for a teacher without a set timetable in your MIS.

Once you have completed the 'Create Feedback' form, click on 'Submit'.

Navigate to Feedback > Create then follow the same process as described above in either the 'Providing feedback on a timetabled lesson for an individual teacher' or 'Providing feedback on a non-timetabled lesson for an individual teacher' sections.

If you are already on a 'Teacher Overview' page, click on the 'Start Feedback' button in the 'Quick Feedback Actions' box.

Top tips 💡
  • Providing feedback to teachers with no set timetables: Schools often ask if they can provide feedback for staff groups such as cover supervisors, long-term agency teachers, teaching assistants and peripatetic teachers. This is easily done in your platform and if any staff in these groups are external, they can be added through Settings > User Management > Create User. See this User Management article for set up instructions.
  • Creating new subjects in your platform: It is possible there are subjects that do not exist in your platform, for which you would like to give feedback, for example, enrichment activities, interventions (for example, phonics or reading), tutor time or even assemblies. Setting up a new subject is a really quick process using the 'Subject' section of this Feedback Management article.
